Every student-athlete must submit a new TISD Physical and all RankOne digital forms for the 2026-27 school year.

Through our partnership with Houston Methodist, Tomball ISD will be offering low-cost, high-quality pre-participation physical exams on two dates.
Tomball High School will be hosting physicals on Saturday, March 28th from 9:00 am - 1:00 pm
Tomball Memorial High School will be hosting physicals on Saturday, April 11th from 9:00 am - 1:00 pm 

Physicals are $21 - Appointments are required and must be purchased online (instructions below). CASH WILL NOT BE ACCEPTED

The instructions outlined below will guide you in completing all forms correctly and ensure your student-athlete is eligible for participation in all UIL activities.

1. Click HERE to create or use your login on RankOne and select the “Student Forms” dropdown.

2. Complete and electronically sign ALL Electronic Documents
        *Medical History
*TISD Random Drug Testing Form
*TISD Pre-Participation Forms
*UIL Forms Signature Page

3. “Download and Print” the 2026-27 Tomball ISD Physical Form. No other form will be accepted. Accuracy on this form is important!  Please read and answer all questions honestly and sign at the bottom of the Medical History page.  The Healthcare Provider will complete the Physical Examination page and should sign/stamp and date the form. 

4. Click HERE to select and purchase your appointment time at TMHS or HERE for THS. When checking out on SchoolCash, please make sure you uncheck the "myself" box and type the First and Last name of the student getting the physical (no nicknames) in the box provided.

5. Click HERE to print, complete, and sign the Houston Methodist Permit Form

ON PHYSICAL DAY: Arrive 10 minutes before your appointment time for check-in at the Athletics Parking lot of Tomball Memorial High School. Make sure you bring:
Houston-Methodist-Permit-Form
Tomball ISD Physical Form (with parent sections completed)
Proof of Appointment/Physical Purchase

6. High school students should turn in their completed Tomball ISD Physical Form to the TMHS campus athletic trainers. Junior high students should turn their completed form into their respective team coach.
